PERSONAL DATA PROTECTION LAW (KVKK) INFORMATION AND CONSENT TEXT

Hello there,

Futureheal may need to learn your personal information and health data and record and store them within the limits required by the service to be provided so that the health services he will provide to you can be carried out properly. 

Your health data, which we have to record to provide health services to you, is considered as special personal data by law. In this context, since personal health data can only be recorded with the explicit written consent of the person, except for the special conditions specified in the law, according to the provision ‘Processing sensitive personal data without the explicit consent of the person concerned’ in paragraph 2 of Article 6 of the Law on the Protection of Personal Data No. 6698, It is obligatory for you to obtain this consent.  

INFORMATION TEXT 

This consent refers to the patient’s consent (assay results, prescriptions, photographs, and so on) obtained in various centers or our practice, which you transmit to us via the internet, mobile applications, or electronically, along with the personal data you provided us verbally, in writing, visually, or electronically during our examination. Your data is protected by video, camera recordings, and other means.

Your data will be recorded only to the extent necessary for the health service to be provided to you, by Personal Data Protection Law No. 6698 and applicable legislation, and will be stored in our system/archive for “not more than the time necessary to fulfill the purposes of recording.” Your processed data will be treated as a professional secret, with confidentiality guaranteed, and will not be shared with third parties, institutions, or organizations.

Requests for the transmission of your data to public institutions, judicial authorities, and other official authorities, the purpose of the request, whether the requested data overlaps intending to be achieved, whether it can be stated concretely, and whether the only way to achieve the stated goal is to transmit your data without anonymization. Data transmission requests that do not satisfy all of these criteria will not be fulfilled, and data transmission requests that do not meet all of these elements will be denied. 

VI. Following the Convention on the Protection of Individuals against Automatic Processing of Personal Data (Council of Europe Convention No. 108), Article 8 of the European Convention on Human Rights, Article 20 of the Constitution, and Law No. 6698 on the Protection of Personal Data, we have collected and stored your data:

I have read and understood Futureheal’s Personal Data Disclosure and Consent Text, and I have received verbal instruction on the issue.

My rights to protect my data, the mandatory conditions under which my data can be transferred, and my data security and application rights, as detailed in the Personal Data Disclosure and Consent text, have been informed of the purposes of the processing, collection methods, and legal reasons for my data, as well as my data security and application rights.

All of my data, including my health data, are recorded, stored, and shared in mandatory instances by Futureheal, per the following standards.

In addition, if my medical diagnosis and treatment demand it, my doctor will share my information with colleagues for consultation or with product supply firms if a unique product is needed for me.

Additionally, Futureheal has the ability to send me communications by mobile devices, e-mail address, or mail address, among other things. It is with my express permission that I accept it.
